Social Worker (MSW) - Mother Baby Unit
Weekly Hours: 24
Status: Part Time
As a member of the treatment team, collaboratively assesses, plans, implements, coordinates, monitors, and evaluates the services and options to meet the needs of the patient. This is accomplished through advocacy, clear communication, and resource management to obtain quality interventions and outcomes.
Principal Duties and Responsibilities:
Utilizes critical thinking and clinical skills to continually assess, plan, implement, coordinate, monitor, and evaluate the needs/goals of the patient.
Actively participates in treatment team, family conferences, and discharge planning.
Determines patient eligibility/need for financial and/or community assistance and refers to appropriate agencies.
Ensures hospital compliance with regulations governing discharges to appropriate levels of care and/or transfers to other hospitals.
Recognizes indicators of abuse, neglect, and exploitation of children and adults, ensures appropriate reporting to authorities and works in cooperation with protective service agencies to address medical and emotional needs of victims.
Develops and maintains good working relationship with treatment team, hospital, and medical staff, and personnel from community health, welfare, and social agencies.
Utilizes EPIC in an effective manner to document in a clear and concise manner.
Works collaboratively with the Women's Health Program Coordinator to ensure care of patients experiencing Perinatal Loss.
- Works collaboratively with the Women's Health Program Coordinator, adoption agencies, and attorneys to ensure proper care and documentation of adoption cases.
Minimum Education, Training and Experience Required:
- Master of Social Work required.
- CSW State Licensure required.
- Experience in Mother Baby/NICU setting preferred.
- Must be able to understand medical terminology/care practices and principles.
- Must have excellent communication skills.
