The primary responsibilities of this position are to provide case management to elderly and physically disabled individuals under the Home and Community Based Services Program. Duties include performing intake and assessments, developing and implementing individualized care plans, providing monitoring and ongoing case management for clients, making referrals to community partners and completing all required training. Duties may also include providing information about Home & Community Based Services to individuals and to community partners.
Require licensure as a Licensed Baccalaureate Social Worker (LBSW) by the ND Board of Social Work Examiners; and one year of professional human services work experience as a social worker, human relations counselor, developmental disabilities case manager, mental illness case manager, vocational rehabilitation counselor, activity therapist, addiction counselor, registered nurse, employment counselor, or a similar professional level position in the public or private sector, or a Master's Degree in social worker and licensure as a Licensed Social Worker by the ND Board of Social Work Examiners.
Individual must be proficient in use of a computer, as well as possess good time management and organizations skills. This position requires the ability to visit clients in their own homes or in facility settings and will involve travel to be able to conduct on-site assessments with clients.
