DETAILS OF THE POSITION YOU ARE APPLYING FOR!
The field RN is a key member of the home health care team responsible for provision of direct patient care to assigned patients and coordination of that care with other professionals. The RN establishes home health aide care plans and supervises the home health aide performing patient care. Additionally, the RN educates the patient and/or caregiver to help them obtain an optimal level of health.
Duties to include:
Perform initial evaluation and periodic re-evaluation of patient's condition and recommend revisions of the patient's plan of care as needed.
Provide services requiring substantial and specialized nursing skill.
Coordinate services and inform the primary physician and designated personnel of significant changes in patient's condition and needs.
Provide or supervise skilled nursing services in accordance with physicians orders.
Develop, initiate, implement, and update the plan of care as needed or at least every 62 days.
Initiate appropriate preventative and rehabilitative nursing procedures as appropriate for the patient's care and safety.
Counsel the patients and family in meeting nursing and related needs.
Manage nursing and multi-disciplinary services for specific patients; including identification of needs, and referrals to other disciplines as appropriate and ordered by physician.
Supervise the care provided by the LVN/LPN to their assigned patients.
Prepare clinical notes in accordance with professional and agency specific standards.
Supervise the care provided by home health aides to their assigned patients.
Contribute to the orientation and ongoing education of other clinical personnel.
Participate in performance improvement activities and mandatory case conferences. Licensed nurse in the state in which they are performing duties
Possess knowledge of Medicare and Medicaid guidelines essential to Home Health Coverage
Demonstrate proficiency in clinical documentation
Possess good organizational and time management skills
Possess strong verbal, written communication and interpersonal skills
Display initiative and the ability to work independently
Possess good problem-solving abilities
Current driver's license with auto liability insurance
Computer, internet access and email capabilities from home or other off-site location
Graduate of an approved vocational school of nursing.
Training and Experience
One (1) year clinical experience as a registered nurse
Home health care nursing experience preferred
Work is performed in patient homes, the employeeâs vehicle, and in an office environment.
Work requires continual mobility.
Frequently sits, stands, stoops, twists, turns, reaches, and grasps.
Continually communicates in person and over telephone systems with patients, staff, and other health care professionals; this requires good hearing and speaking skills.
Close-up vision is frequently required.
Frequently exposed to the hazards of city driving and is often exposed to temperature extremes.
Sometimes exposed to potentially unsafe and unsanitary conditions in patient homes.
Frequently exposed to biohazards in performing patient care activities.
Frequently pushes and pulls a typical weight of 100 pounds; maximum push/pull weight is 250 pounds.
Frequently lifts and carries a typical weight of 10 pounds; maximum lift/carry weight is 50 pounds.
