Integrative Case Manager is responsible for providing routine standardized self-sufficiencyassessments, service plans, supportive services, information and referral, life skills training, advocacy and aftercare services to case management clients in the program. Also responsible for maintaining case files in accordance with agency, client and funder policies/ procedures and gathering and reporting program statistical and outcome data. This position works closely with homeless clients, other organizations and programs within the agency to provide comprehensive services and assist clients/families during the healing process as families move toward an improved sense of well-being and self -sufficiency. The successful candidate will be comfortable working with the relevant community partners and especially those working to identify suitable rental units and willing landlords, will provide case management services with appropriate empathic and empowering clinical skills, will be familiar with grant funded positions and requirements and will participate in required training specific to program requirements, the homeless population and trauma related issues. Finally, candidate should be a team player and willing to embrace the mission and vision of the larger client agency.
ESSENTIAL JOB DUTIES
- (50%) Provide direct client services including, but not limited to: rapport building, client Intake and eligibility determination, completing monthly standardized assessment, evaluate data for individual client progress, develop individualized service planning in response to assessments, identify and coordinate appropriate services (inside and outside of the agency), provide client advocacy, complete case monitoring and provide routine aftercare services. Participate in weekly service plan meetings and case consultation services.
- (20%) Maintain accurate and timely documentation in client files in compliance with agency, clientand funder policies and guidelines including: intake, standardized self-sufficiency assessment, service plans, case notes, quarterly reviews, revision of service plans and other documents as required by funding sources and agency. Ensure the proper security of client records and confidentiality in compliance with HIPAA and agency policy. Participate in quality review of client records for compliance with all required standards.
- (10 %) Collaborate with other social service and social justice advocacy agencies that assist those who are homeless or at-risk of being homeless such as the Homeless Coalition, local shelters, legal aid service providers and others as part of a holistic approach to service provision
- (5%) Provide Financial Assistance to clients per grant requirements to ensure access to basic needs including help with shelter, clothing, food and transportation issues essential for restoration
- (10%) Provide Emergency and Crisis-Focused case management
- (5%) Perform other duties as assigned
MINIMUM EDUCATION REQUIREMENTS
- Licensed Social Worker - Master's Degree preferred; obtain LMSW license within 90 days of hire.
MINIMUM SKILLS AND EXPERIENCE REQUIREMENTS
- Bilingual in English and Spanish
- Ability to respond compassionately to persons who are in crisis, managing illness or are victims of domestic violence and other traumatic events, while maintaining appropriate and professional boundaries.
- Knowledge of general social work practice and specific knowledge of trauma and domestic violence
- Minimum of two years of experience with case management
- Knowledge of community resources, particularly those related to the homeless population
- Ability to work with diverse populations in crisis situations
- Ability to practice and maintain client/program confidentiality, while at the same time function as a collaborating staff/team player
- Strong interviewing and assessment skills
- Strong verbal and writing skills, analytical, conceptual, problem-solving and independent decision making skills
- Proficiency in basic computer skills including WORD, EXCEL, OUTLOOK
