Mental Health Skill-building Counselor

The
MHSS Counselor is responsible for providing goal-directed training
to clients with serious mental health diagnoses and challenges, and enables
these clients to achieve stability and independence in a less restrictive
community based environment. The Community Based Counselor provides training
and skill building to clients in areas including medication management and
knowledge of side effects, coping skills, safety precautions, appropriate
personal and interactive behavior, activities of daily living, use of community
resources, and overall monitoring of health and well-being.

The MHSS Counselor will develop, maintain, and effectively implement
client's Individualized, Service Plan (ISP); develop, implement, and
review client discharge plans; produce high quality, punctual, and accurate,
documentation of all casework consistent with licensure and agency
policies; maintain thorough records of all services provided; attend
and participate in treatment team meetings, coordinate care to ensure
client's health challenges are addressed; and participate in individual
supervision.

Minimum Requirement
Qualified Mental Health Professional (QMHP-A): A minimum of a Bachelor’s degree in a human services field and at least one year clinical experience providing direct services to individuals with mental health diagnoses.
Or, Qualified Paraprofessional in Mental Health (QPPMH): An Associate’s degree and at least 1 year clinical experience providing direct services to individuals with mental health diagnoses or 90 hours of classroom training, and 1 year experience under the direct supervision of a QMHP-A providing direct services to individuals with mental health diagnoses .

Must have reliable transportation
