POSITION SUMMARY Provides clinical social work interventions with oncology patients and families at the Solinsky Center for Cancer Care in Manchester and Londonderry to include Medical Oncology,#Radiation Oncology and Breast Health services#that address personal and environmental issues in order to maximize emotional, social and physical well-being and the appropriate and efficient use of health care services across the continuum. Promotes efficient coordination of patient care through collaborative practice with the health care team. Promotes patient, family, staff and physician satisfaction through coordination and continuity of care. Provides a systematic approach to evaluation of patient care. Coordinates patient care through the continuum thereby facilitating the achievement of optimal quality outcomes in relation to clinical care. Provides ad-hoc support services and support groups to patients at the Elliot Breast Health Center. KEY RESPONSIBILITIES Collaborates and participates with the primary nurse and the multidisciplinary team in the ongoing clinical social work assessment of each patient. Identifies relevant community resources and links them to patients as appropriate. Provides supportive counseling and crisis intervention for patient/family as needed. Coordinates with key personnel at all community health facilities, which treat oncology patients, to ensure patient needs are met. Assists the Health Care Team in the coordination of patient care by serving as a resource to staff and assisting in day to day psychosocially based problem resolution. Collaborates with the American Cancer Society to sponsor ACS programs within the hospital setting. Provides counseling and assistance with Advance Care Directives. Advocates for the patient to ensure quality outcomes. Coordinates and facilitates family meetings. Respects the dignity and confidentiality of the patient and all verbal/written information/communication. Develops and conducts Education/Support Groups for patients/families based upon the needs of the patients. Provides emotional support to patient/family to facilitate adaptation to illness/disability. Addresses financial concerns which may impact optimal patient care. Assesses patient/family adaptation to illness/disability and capacity to provide for patient#s care needs via biopsychosocial assessment. Serves, as a resource/liaison to community social service agencies relative to identified patient/family needs. Collaborates with community social services agencies to enhance continuity of care. Facilitates and coordinates non-medical/social/cultural resources, including but not limited to Hospice, Nursing Home Placement, Homemakers and Community based Social Services. (i.e. HCBC, Adult Day Care, In Home Adult Care, Companion Program, Emergency Responses Systems etc). Coordinates homecare referrals, acute rehab, and nursing home referrals as defined. Conducts assigned quality management activities to improve patient care and service. Participates as part of the interdisciplinary team in the development or revision of hospital policies /procedures pertaining to patient care processes. Participates in multidisciplinary meetings to review activities/outcomes/issues related to assigned unit. Maintains current knowledge of: Available community resources, Current trends and research in oncology social work and regulatory standards i.e. State, Federal, DNV and COBRA. Participates in relevant professional activities and organizations and shares information with peers. Participates in committees. Participates in appropriate role in fire and disaster drills, fires and other emergency situations. Exercises care in the use and operation of equipment and reference materials. Participates in department in-services, activities, educational opportunities and training programs, which promote professional development. Performs similar or related duties as assigned or directed. REQUIRED KNOWLEDGE # SKILLS Ability to deliver patient care in a manner consistent that is appropriate to the patient#s age, physical ability and intellectual development. Must demonstrate proficiency in assessing treatment and responses and adapting care to meet the needs of the patient population served; pediatric, adolescent, adult and geriatric. Ability to work with frequent interruptions and respond appropriately to unexpected situations. Familiarity with information systems design and development. Ability to transport self to and from off-site locations. Ability to plan and perform diversified duties requiring an extensive knowledge of a particular field and the use of a wide range of procedures. Involves the exercise of judgment in the analysis of facts and conditions regarding individual problems or transactions to determine what actions should be taken, within the limits of standard practice. Ability to work with other members of the healthcare team and community to provide the best comfort, caring and curing. EDUCATION/EXPERIENCE/LICENSURE Education: Master of Science in Social Work required.# Experience: One (1) to three (3) years of experience working with oncology patients and cancer survivors.
